1. Listen to the online music samples and select your choices on the form - the titles, composers, and hymn numbers (if noted).
2. Call the cantor that you would like to use directly. If you choose to have a solo instrument player, Daniel F. Kovacic will contact a professional player for the ceremony.
3. If you are having a program printed and want to list the titles of these pieces, complete and return the form and I will call you to suggest musical options for these slots.
4. Fill out the form in its entirety. Some questions are required answers. Once completed, please fill out the form, print out a copy for yourself, and bring it to our face to face meeting. You can send the form to:

The Responsorial Psalm is a required sung part of the liturgy. This part is not recited. Therefore, ignore the choices in the "Together for Life" book for the Responsorial Psalm. Choose from one of the examples listed above.
Instruction/Consent: Vows and Exchange of Rings
The Lighting of the Unity Candle is an optional part of the wedding ceremony. This is not a part of the official Rite of Marriage. No special music accompanies this action since it is brief.
The Lord's Prayer: Recited. If you want the celebrant to chant the Lord's Prayer, you must discuss it with him prior to the wedding.
The Sign of Peace: I will provide soft accompaniment during this part of the Mass.
*The selections marked with an asterisk are OMITTED if there is no Mass.
